Hamilton Public Health form for food vendors

Please complete the required Hamilton Public Health Form below and return to this email at least 21 days prior to the event date.

DO NOT EMAIL TO HAMILTON PUBLIC HEALTH UNIT.

 

Please don't forget to also include the following: 

  1. Your kitchen’s most recent public health inspection report. (Not the GREEN poster, we need the actual inspection report)

  2. If you rent a kitchen space to prepare food, you MUST attach a letter from the owner of the kitchen space confirming that the food is prepared at that location. A copy of the kitchen’s most recent public health inspection report shall be included.
