Public Health forms for food vendors

IMPORTANT! Send forms to  jackie@theartisanbazaar.ca

 

Please complete the required Hamilton Public Health Form below and return to the above email no later than October 22, 2025. If forms are not completed in full and with correct information, a $35 fee may apply. Please make sure all required info is included.  If forms are sent after October 29, a late fee of $35 will apply.

The following MUST be included:

1. Your kitchen’s most recent public health inspection report. (Not the GREEN poster, we need the actual inspection report)

2. If you rent a kitchen space to prepare food, you MUST attach a letter from the owner of the kitchen space confirming that the food is prepared at that location. A copy of the kitchen’s most recent public health inspection report shall be included.
