Public Health forms for food vendors

Food Vendor Document Submission

As part of our participation requirements, all food vendors at The Artisan Bazaar must submit their completed Public Health Form and/or Kitchen Inspection Report in advance of the event. This ensures compliance with local health regulations and a smooth check-in process on event day.

How to Submit:

  1. Download the required form(s). Links are provided within the submission form.

  2. Complete the form(s). Most devices allow you to “Save As PDF” when printing or scanning.

  3. Upload your completed PDF(s) using our secure Vendor Submission Form 

 

Important Notes:

  • Only PDF files will be accepted. Photos, Word documents, or screenshots cannot be processed.

  • Documents must be received no later than October 17, 2025. Submissions after this date may not be accepted/may be subject to a $35 late fee, and failure to provide documents could affect your participation in the event.
